All 5 can be avoided if the plumber in question has been properly trained.

I don't believe there was a permit on the job with the cleanout above the electrical panels. In addition to the cleanout location, the plumber also joined PVC to ABS without a transition coupling. Not allowed here in Colorado.

Did not catch that transition! Good eye.

Funny thing, when transisitioning from vertical down flow to horizontal flow condition, both codes require the use of a wye and 1/8th bend on its back to make this transition. I had a job once where we had to replace 100's of these combination fittings, and each and every one of them had a significant build up of solids on the horizontal drain where the vertical tied in. Only way around it would be to require side streams to enter from the side of the horizontal drain pipe, which would add a 90 degree sweep fitting to the package. Bottom line, what happens in the field, is sometimes different than what happens in the minds of plumbing engineers. I should have taken pictures of the build ups and shown them to the technical committee members who put these codes together.

Mark,
I so agree, as a contractor in the field for decades I see things that are problematic but I don't do well presenting TA's. Several methods of pipe transition and the need for the pressure reducing valve to be at the meter if one is needed. Other issues exist still, but we do still have the best @ IAPMO.

The most frequent code violation I see in Ca. is no pan under water heaters, an if one is there, no drain line. Also thermal expansion tanks not present on water heaters. Next is missing air gaps for DW.
